Case Management

A large part of the work our nurses undertake is the provision of Case Management. This entails assisting clients who would otherwise be unable to manage day to day requirements of daily living. It may be negotiating respite or short term admission to care, assisting with funding applications for equipment, liaising with services on the client’s behalf, assisting with referrals to other services or communicating with other service providers. Having complex health needs can often mean you have multiple services proving care for you. The role of case manager is to coordinate all of the input from each service to make sure the client gets the best possible outcome.  Case management can occur on a short term basis or as an ongoing service.
